How do I submit a claim? (Accidental Death)

The steps for submitting a claim (Accidental Death) are stated as follows:

1. Visit the IglooCare portal here: https://igloocare-my.iglooinsure.com/login
2. Login with your registered mobile number and key in the One-Time Password (OTP).
3. Select the item(s) you wish to claim.
4. Set the Date of the Incident and click "Submit".
5. Under the 'Basic Information' page, do the following:
● Input Order ID and Date of Incident
● Select "Myself - Policyholder" under 'Who is this claim for?'
● Select "Accidental Death" under 'Claim Type'
● You will be prompted with a confirmation pop-up, then click "Confirm"
● Click "Next"
6. In ‘Authorized Documents’, provide the following documents:
● Claim Form
● Copy of Malaysian Identification Card (IC) or Passport
● Police Report (Declaration of Death)
● Diagnosis Letter from Doctor (Cause of Death)
● Proof of Product Purchase (Shopee invoice)
7. In Account Information for Reimbursement, select your preferred payment method of reimbursement whether by ShopeePay or Bank Account.
8. In Review & Declaration, tick the box "By proceeding, I agree to Igloo's Terms of Service and Privacy Policy" and click "Submit".
9. You will be directed to a screen with the notification status "Your claim has been submitted successfully!".


Bagaimana saya mengemukakan tuntutan? (Kematian Akibat Kemalangan)


Langkah-langkah untuk mengemukakan tuntutan (Kematian Kemalangan) adalah seperti berikut:

1. Lawati portal IglooCare di sini: https://igloocare-my.iglooinsure.com/login 
2. Log masuk dengan nombor telefon mudah alih yang telah didaftarkan dan masukkan Kata Laluan Satu Kali (OTP).
3. Pilih item(s) yang ingin anda buat tuntutan.
4. Tetapkan Tarikh Kejadian dan klik ""Hantar"".
5. Di bawah halaman 'Maklumat Asas', lakukan perkara berikut:
● Masukkan ID Pesanan dan Tarikh Kejadian
● Pilih ""Saya - Pemegang Polisi"" di bawah 'Siapakah tuntutan ini untuk?'
● Pilih ""Kematian Kemalangan"" di bawah 'Jenis Tuntutan'
● Anda akan diminta dengan pop-up pengesahan, kemudian klik ""Sahkan""
● Klik ""Seterusnya""
6. Dalam 'Dokumen Diluluskan', sediakan dokumen-dokumen berikut:
● Borang Tuntutan
● Salinan Kad Pengenalan Malaysia (IC) atau Pasport
● Laporan Polis (Pengesahan Kematian)
● Surat Diagnosis dari Doktor (Sebab Kematian)
● Bukti Pembelian Produk (faktur Shopee)
7. Dalam Maklumat Akaun untuk Bayaran Balik, pilih kaedah pembayaran yang anda kehendaki, sama ada melalui ShopeePay atau Akaun Bank.
8. Semak & Perisytiharan, tandakan kotak ""Dengan meneruskan, saya bersetuju dengan Terma Perkhidmatan dan Dasar Privasi Igloo"" dan klik ""Hantar"".
9. Anda akan dibawa ke skrin dengan status pemberitahuan ""Tuntutan anda telah berjaya dihantar!"".
